CHAPTER ONE: The Wet Nurse and the Well: Early Feeding and Infant Mortality

For almost the entirety of human history, an infant denied its mother’s milk stood on the edge of a precipice. Before the advent of modern food chemistry, refrigeration, and germ theory, the biological equation of early human survival was brutally simple: a baby drank breast milk, or a baby died. There were few middle grounds and virtually no safe substitutes. In eighteenth-century Europe and America, roughly one in every three infants died before reaching their first birthday. In crowded industrial cities and foundling hospitals, that figure regularly climbed to seventy, eighty, or even ninety percent. To look into a cradle in the year 1800 was to look at a coin toss between life and early burial.

When a biological mother could not nurse—whether due to death in childbirth, systemic illness, insufficient lactation, or the practical demands of factory or agricultural labor—society had only two options for keeping the infant alive. The first was to find another woman to suckle the child. The second was "dry feeding," a euphemism for hand-feeding the baby a slurry of animal milk, flour, water, and broth using crude, unsterilizable vessels. The first option was a complex, expensive social bargain fraught with moral panic and economic inequality. The second option was very often a death sentence.

The practice of wet nursing—hiring a lactating woman to feed another person’s child—is as old as recorded civilization. It appears in the Code of Hammurabi, the Old Testament, and classical Roman legal texts. For centuries, it functioned as both an essential medical intervention and a stark status symbol. Among the European aristocracy and upper classes, maternal breastfeeding was frequently viewed as distasteful, physically ruinous, or inconvenient to social obligations. A wealthy woman was expected to maintain her place in society and remain available to produce further heirs, an expectation aided by the fact that lactational amenorrhea—the natural suppression of ovulation during regular breastfeeding—was effectively bypassed by outsourcing the task.

This created a thriving, highly stratified market for human milk. At the top of this market were domestic wet nurses, who lived in the home of the wealthy family. These women were subjected to intense scrutiny. Medical handbooks from the Renaissance through the nineteenth century offered exhaustive guidelines on how to select the ideal nurse. Physicians advised noble families to inspect a candidate’s teeth, skin, moral character, temperament, and even the shade of her hair, under the pervasive belief that a woman’s physical and moral vices could be transmitted to the infant directly through her milk. A nurse deemed too passionate, melancholic, or fondness-prone might corrupt the child's spirit, while a nurse who ate the wrong vegetables might sour the baby’s blood.

For the nurse herself, the transaction was deeply fraught. To sell her milk to a wealthy family, a woman usually had to be a new mother who had either lost her own child or was forced to abandon it to a parish workhouse or foundling home. The economic reality was agonizing: the wages earned by nursing a rich family’s child could mean the difference between starvation and survival for her own family, but the physical reality required her to relinquish or neglect her own offspring. In many European cities, this led to a tragic cascading effect, where the survival of an affluent child came at the direct expense of a poor child’s life.

By the eighteenth century, wet nursing had expanded far beyond the aristocratic elite, becoming an organized industry across Western Europe. In France, the bureau des nourrices (bureau of wet nurses) regulated a massive rural placement system. In Paris, thousands of working-class mothers—shopkeepers, laundresses, seamstresses, and domestic servants who worked long hours and could not keep an infant at their side—sent their newborn babies away to the countryside to be nursed by peasant women. Every year, thousands of infants were loaded onto crude, horse-drawn wagons and shipped out to rural regions like Normandy or Burgundy.

The conditions of these rural placements were often horrific. Country wet nurses were frequently impoverished, overworked, and burdened with too many infants at once. A single nurse might attempt to feed three or four babies simultaneously, supplementing her declining milk supply with dirty canal water or boiled flour. Unsurprisingly, thousands of city infants never returned home. In the late eighteenth century, official French statistics revealed that out of roughly 21,000 infants born in Paris each year, nearly 18,000 were sent out to rural nurses. More than half of those sent away died before returning.

The social panic surrounding wet nursing grew steadily throughout the eighteenth and nineteenth centuries. Moralists accused upper-class women of unnatural coldness, while social reformers targeted the mercenary nature of hired nurses. In Great Britain and the United States, sensationalist newspapers published exposes on "baby farming"—a practice where women accepted lump-sum payments to take in infants under the guise of nursing them, only to neglect them, feed them sleeping draughts loaded with opium, and allow them to starve so they could collect the fee and accept another child.

Yet, despite the moral condemnation and emotional hazards of wet nursing, the alternative was vastly worse. If a family could not afford a wet nurse, or if a child was abandoned to public charity, the infant was subjected to "dry feeding" or "hand feeding." This meant attempting to nourish a fragile human digestive system using whatever liquids and pastes were readily at hand.

The standard menu for a hand-fed infant in the pre-industrial world consisted primarily of "pap" and "panada." Pap was a simple mixture of flour, meal, or breadcrumbs boiled in water, broth, or occasionally milk until it formed a thick paste. Panada was a similar concoction made by boiling stale breadcrumbs in water or wine, sometimes seasoned with a little butter, salt, or sugar. To modern eyes, these mixtures seem absurdly unsuitable for a newborn, and physiologically, they were disastrous. An infant under six months lacks sufficient quantities of salivary and pancreatic amylase, the enzymes required to break down complex starches. Feeding a newborn a diet of heavy, starch-laden bread slurry caused severe indigestion, painful gas, intestinal inflammation, and chronic diarrhea.

When infants were fed animal milk—typically cow, goat, or sheep milk—it was rarely given raw or unadulterated. Because raw milk spoiled rapidly in the absence of refrigeration, caregivers often diluted it with warm water, sweetened it with molasses, or mixed it into pap. What they did not realize was that animal milk and human milk possess radically different chemical structures. Cow’s milk contains significantly more protein—specifically hard-to-digest casein curds—and far less sugar (lactose) than human breast milk. When a newborn’s stomach encounters undiluted cow milk, the high casein concentration forms a dense, tough curd that the immature stomach struggles to break down, leading to vomiting, severe intestinal distress, and malabsorption.

Without understanding protein ratios or vitamin chemistry, caregivers watched hand-fed infants wither away from malnutrition even as their stomachs were stuffed with pap. Scurvy (caused by a lack of Vitamin C) and rickets (caused by a lack of Vitamin D) were rampant among hand-fed infants. Children suffered from chronic failure to thrive, their bones softening, their skin breaking into rashes, and their immune systems collapsing under the weight of constant dietary distress.

Yet, as terrible as the nutritional deficiencies of pap and animal milk were, they were not the primary driver of the staggering infant mortality rates of the era. The true killer was invisible, residing on the surfaces of the utensils used to feed the babies and in the water used to mix their food.

Before the widespread acceptance of Louis Pasteur’s germ theory in the late nineteenth century, the concept of bacterial contamination did not exist in the public consciousness. Cleanliness was viewed through the lens of visible dirt, smell, and aesthetics rather than microscopic sanitation. The implements used to feed infants in the seventeenth, eighteenth, and early nineteenth centuries were, by modern standards, death traps.

For centuries, the primary tool for hand-feeding was the "pap boat"—a small, boat-shaped vessel made of pewter, silver, bone, or earthenware, featuring a elongated spout or lip. Pap or milk was poured into the boat and tipped directly into the baby’s mouth. Another popular implement was the "sucking horn," constructed from a hollowed cow’s horn with a small hole drilled in the tip. To soften the flow of liquid and mimic a nipple, caregivers would lash a piece of rolled parchment, a dried cow's teat, or a small piece of rag or sponge to the end of the horn.

These devices were impossible to clean effectively. Pewter and earthenware vessels frequently possessed rough, porous interior surfaces and narrow seams where food particles lodged and rotted. Cow horns were difficult to scrub out and deteriorated over time. The rag, sponge, or leather teats tied to the tips were catastrophic; they remained damp, soaked up milk proteins, and harbored booming colonies of lethal bacteria. Caregivers rarely boiled these implements, often merely wiping them with a greasy kitchen cloth or rinsing them with unboiled well water between feedings. A single sucking horn could be used for days on end, accumulating a crust of decaying milk and starch that was served directly to the child with every meal.

In the early nineteenth century, glass feeding bottles began to gain popularity. While glass was smoother and easier to inspect visually than pewter or horn, early bottle designs introduced new and even more deadly mechanics. One of the most infamous was the "banana bottle" or tube bottle, introduced mid-century. This glass bottle featured a long, flexible rubber tube running from the cap inside the bottle out to a glass mouthpiece tipped with a rubber teat.

The design was hailed as a brilliant convenience for mothers and nurses. Because the tube allowed the infant to lie in a crib and suckle from the bottle on its own, it freed mothers from having to hold the baby during long feedings. Millions of these bottles were sold across Europe and North America. However, the rubber tubing was extremely narrow and porous. It was virtually impossible to scrub the inside of the long tube, and hot water alone could not remove the film of milk that dried inside it. The tubing quickly became a dark, foul-smelling breeding ground for lethal pathogenous bacteria, including Escherichia coli, Salmonella, and Streptococcus. Unaware of the danger, parents watched their babies drink from these "indispensable" bottles, only for the children to fall violently ill hours later. These implements were so deadly that they eventually earned the grim moniker "killer bottles" among early public health advocates.

The liquid used to dilute pap or fill these bottles was equally hazardous. In nineteenth-century cities, municipal water supplies were frequently contaminated with human and animal sewage. Urban families pulled water from shallow neighborhood wells located mere feet from privy vaults, unlined cesspools, and manure piles. When a mother boiled water to make pap, she might briefly reduce the bacterial load, but just as often, water was added unboiled to warm milk to bring it to feeding temperature.

The result of this toxic combination of spoiled milk, filthy vessels, and contaminated water was an annual summer massacre. Every year, as the weather warmed between June and September, cities across the Western world were hit by a wave of severe, epidemic infant diarrhea, known colloquially as "summer complaint," "cholera infantum," or "bowel hives."

Summer complaint was a terrifyingly swift illness. A baby who appeared perfectly healthy in the morning could develop severe, watery diarrhea and persistent vomiting by the afternoon. Within twenty-four to forty-eight hours, the child would succumb to profound dehydration, high fevers, and hypovolemic shock. Because doctors of the era did not understand that the illness was caused by bacterial infections transmitted through food and water, their medical interventions were useless, and often counterproductive. Physicians prescribed mercury-based purgatives like calomel, bleeding, or heavy doses of astringents and opiates like laudanum, which only accelerated the infant's decline.

In the summer months, the death tolls in major cities reached apocalyptic proportions. In New York, Philadelphia, London, and Berlin, infant mortality spiked so sharply during hot weather that local governments struggled to keep pace with small burials. In the 1840s, it was estimated that up to half of all infants hand-fed with cow's milk in urban centers died of bowel infections before reaching twelve months of age.

Nowhere was this carnage more visible than in the foundling hospitals of Europe. Established during the Enlightenment as charitable sanctuaries for unwanted, abandoned, or orphaned infants, these massive institutions quickly became testament to the impossibility of hand-feeding infants at scale without modern science.

The London Foundling Hospital, established by Thomas Coram in 1739, and the Hôpital des Enfants-Trouvés in Paris received thousands of infants every year. Mothers left their newborns in baskets at the gates or passed them through rotating wooden barrels (tours) set into the hospital walls, desperate to give their children a chance at life when they could not afford to raise them.

The institutions were quickly overwhelmed. With hundreds of infants arriving every month, it was impossible to hire enough wet nurses to feed them all. The hospitals were forced to rely on dry feeding. Teams of nurses worked in overcrowded, drafty wards, feeding dozens of babies at a time from shared pap boats and wooden spoons filled with boiled flour and sour cow’s milk.

The mortality rates inside these institutional walls were horrifying. At the Dublin Foundling Hospital between 1775 and 1796, over 10,000 children were admitted; less than three hundred survived, representing a death rate of over ninety-seven percent. In the Foundling Hospital of St. Petersburg, Russia, thousands of infants perished annually from dysentery, marasmus, and respiratory infections. Rather than saving lives, these charities had inadvertently created giant death chambers where gastrointestinal infections jumped rapidly from crib to crib via unwashed hands and shared feeding tools.

Medical writers of the eighteenth and early nineteenth centuries were well aware of the catastrophe, even if they lacked the biochemical framework to fix it. Eminent physicians wrote passionate treatises begging mothers to nurse their own children. In 1748, the English physician Cadogan published An Essay upon Nursing, and the Management of Children, in which he condemned the widespread use of pap, panada, and heavy clothes, advocating instead for clean air, loose clothing, and exclusive maternal breastfeeding. Cadogan accurately identified that the thick, heavy starches fed to infants were causing severe digestive trauma, writing that "the light and dry food, such as dry bread, water gruel, and panada, are utterly unfit for a newborn child, whose stomach is too weak to digest them."

Similarly, in France, Jean-Jacques Rousseau’s 1762 treatise Émile sparked a cultural shift among the upper classes by romanticizing natural motherhood and framing breastfeeding as a fundamental moral duty. Rousseau’s writings made it fashionable for aristocratic mothers to nurse their own children, temporarily reducing the reliance on rural wet nurses among the wealthy elite.

Yet, moral exhortations and philosophical treatises could not solve the underlying structural reality. Maternal breastfeeding was not always physically possible, nor was it always economically viable. A mother dying of tuberculosis, a mother who produced no milk, a mother who worked fourteen-hour shifts in a Lancashire cotton mill, or an orphaned child in a municipal workhouse could not be helped by Rousseau’s romantic philosophy. They required a safe, non-human source of nutrition that could reliably sustain an infant's life.

By the mid-nineteenth century, the crisis had reached a tipping point. The rapid urbanization caused by the Industrial Revolution was pulling millions of women out of traditional agrarian settings and placing them into urban factories and sweatshops. Extended family structures dissolved, taking traditional community support and experienced grandmothers with them. As cities grew denser and dirtier, the distance between the dairy cow and the urban consumer lengthened, meaning milk took days to reach city centers—arriving warm, unpasteurized, heavily diluted, and frequently adulterated with chalk, plaster, or formaldehyde to hide the stink of spoilage.

The early nineteenth-century mother who could not breastfeed was caught in an impossible trap. Wet nursing was economically out of reach for the working poor and increasingly recognized as a vector for moral and medical trauma. Hand-feeding with pap or urban cow’s milk was a game of Russian roulette where the chamber was loaded five times out of six.

The stage was set for a fundamental shift. Humanity had spent millennia trying to substitute breast milk using trial, error, folk remedies, and crude utensils, with devastating results. What was required was a systematic, scientific approach—a break from ancient tradition that would dismantle milk down to its base chemical components, understand the precise metabolic needs of the human infant, and industrialize the production of a safe, shelf-stable substitute. The desperate need for a solution would soon attract the attention of the world’s leading chemists and inventors, triggering the birth of modern food science and transforming the survival of infants from a matter of tragic chance into a multi-billion-dollar scientific enterprise.
