This technique was refined over time, with practitioners sometimes carrying the "pocky matter" at body temperature for a month before use, or even extracting fluid from pustules onto a cotton plug for nasal insertion. In India, similar practices were carried out, often involving the transfer of material from smallpox pustules to the skin of healthy children using a lancet or needle. These early practitioners, often itinerant Brahmins, relied on centuries of accumulated empirical knowledge. The drying of scabs in the sun, as practiced in China, was thought to weaken the virus, making the induced infection less severe. The choice of method and careful selection of source material were crucial, as the goal was to trigger a protective immune response without causing a life-threatening illness.

The practice of variolation gradually spread westward. By the 17th century, it was observed in parts of Africa and the Middle East, particularly the Ottoman Empire. In fact, accounts from the 18th century suggest that some form of variolation had been in use in Wales as early as the 17th century, where smallpox material might be wrapped around a child's wrist or scabs held in their hand. These diverse, culturally embedded practices highlight a universal human drive to combat disease, even when the underlying mechanisms were a complete mystery.

The journey of variolation to Europe and colonial America in the early 18th century is a fascinating tale of cultural exchange and human courage. Lady Mary Wortley Montagu, wife of the British ambassador to the Ottoman Empire, played a pivotal role in introducing variolation to England in 1721.
