However, a true understanding of its pathology remained elusive for centuries. Ancient Greek physicians, for example, recommended horseback riding to reduce excessive urination, while others prescribed diets of milk and barley water. By the mid-17th century, carbohydrate restriction began to emerge as a treatment approach, followed by the "Meat Diet" in the 1700s.

In the late 19th and early 20th centuries, Dr. Frederick M. Allen championed a "starvation treatment" that severely restricted all food components, aiming for an optimum point of malnourishment to sustain the diabetic patient. While this approach could prolong life for a few months or a year, it often presented a grim choice: death by diabetes or death by starvation. The quality of life for these patients was, predictably, extremely poor, marked by constant hunger and weakness. There was no real hope, only a slow and agonizing decline.

The scientific community, however, was not idle. Researchers had long suspected a connection between the pancreas and diabetes. As early as 1869, German pathologist Paul Langerhans had identified two systems of cells within the pancreas. Later, in 1910, Sir Edward Albert Sharpey-Schafer further elaborated on the pancreatic islets' ability to secrete a substance controlling glucose metabolism, which he termed "insulin," derived from the Latin word "insula" for island. These observations, while significant, were still theoretical, and the elusive substance itself remained unisolated and unusable for therapeutic purposes.

The critical breakthrough arrived in the summer of 1921, at the University of Toronto, a moment that would forever change the lives of millions. Dr. Frederick Banting, a Canadian surgeon, alongside his young medical student assistant, Charles Best, embarked on a series of experiments under the directorship of Professor John Macleod. Banting, who had been fascinated by the pancreas's role in diabetes, believed that if insulin could be extracted from the pancreas, it could help those suffering from the disease.

The team soon brought in biochemist James Collip to purify the insulin extract further. Collip's expertise proved invaluable in refining the extraction process, making the substance safe enough for human trials. On January 11, 1922, a 14-year-old boy named Leonard Thompson, gravely ill with type 1 diabetes and weighing only 65 pounds, became the first human patient to receive an insulin injection. His blood sugar levels were dangerously high, and his condition was rapidly deteriorating. The initial dose, still somewhat impure, caused an allergic reaction. Undeterred, the team worked tirelessly to refine the purification process, and just twelve days later, on January 23, 1922, Thompson received a second, more refined dose. The results were dramatic and immediate: Thompson's blood sugar levels dropped, and his clinical condition improved significantly. He regained strength and weight, and the once-fatal prognosis transformed into a manageable chronic condition.

The news of insulin's success spread like wildfire, a beacon of hope in a world where diabetes had been an incurable scourge. The impact was sensational; those who witnessed the first starved, comatose diabetics revived by insulin saw a genuine miracle of modern medicine unfold before their eyes. The University of Toronto, recognizing the immense humanitarian potential of this discovery, made the unprecedented decision to license insulin production to pharmaceutical companies royalty-free.
