Beyond PICOT, another helpful set of criteria for evaluating your research question and objectives is the FINER criteria: Feasible, Interesting, Novel, Ethical, and Relevant. Ethical is non-negotiable. Your study must respect ethical standards and safeguard the rights and welfare of human participants. Once you’ve used these frameworks to refine your research question, you can then translate it directly into your primary and secondary objectives. This ensures that the purpose of each objective is explicitly stated, leaving no room for ambiguity. Historical controls or external controls compare trial participants to data from previously conducted studies or existing databases. Finally, the concept of adaptive control arms is gaining traction. In some adaptive trial designs, the proportion of participants assigned to different treatment arms can change over the course of the study based on accumulating data. For example, if one arm is clearly superior or inferior, fewer participants might be allocated to the less promising arm. This can optimize resource use and potentially reduce the number of patients exposed to ineffective treatments, though these designs introduce statistical complexities.

The meticulous choice of endpoints, the precise definition of eligibility criteria, and the thoughtful selection of a control arm are not merely bureaucratic hurdles.
